Measuring results, Maximizing impact.
Erie County’s Children’s System of Care uses High-Fidelity Wraparound (HFW), an evidence-based intervention model, to improve outcomes for high risk/needs youth and their families. Ideally, the HFW model supports the integration of child-serving systems that allows the family to develop and strengthen its ability to take care of itself. This value-added care initiative is nationwide, and as such, there are national metrics that the countywide impact can be measured against. To provide an accurate picture of how HFW was faring in Erie County, CCNY implemented a comprehensive evaluation program. The program included stakeholder interviews and surveys, direct observation of the evidence-based team process, and analysis of process and outcome measures —all of which was summarized in a detailed evaluation report that answered whether or not High-fidelity Wraparound Care Coordination in Erie County improves families’ outcomes related to safety, permanency and well-being.
