Providing Program Evaluation Services to help Build a Transition Age Youth System of Care

 

CCNY provided program evaluation services for two grants that targeted a specific youth population in need of specialized care and attention. The overall goal for both initiatives is to create a Transition Age Youth System of Care through improved access to treatment and support services for youth and young adults ages 16 – 25, specifically designed to prepare them to transition into adult roles.

CCNY provided comprehensive evaluation and reporting services, required by the Substance Abuse and Mental Health Services Administration, to monitor, track and measure grant activities and outcomes. We went beyond the scope of work to support continued training, data collection and reporting to SAMHSA and the DC Department of Behavioral Health—Ensuring that the grants and the populations they serve were served and sustained.
